Job Purpose

The Day Program Facilitator provides direct support to clients who participate in the Day Program including developing and facilitating groups as well as individual support. She/he documents all information required to determine the client's progress towards the achievement of group and individual participant goals.

 

Duties and Responsibilities

Service Provision

  • Create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan.
  • Understand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed.
  • Transport clients as assigned (able to drive a 12-passenger van).
  • Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk-taking.
  • Make preparations and serve lunch for up to 25 people.
  • Provide crisis intervention, conflict resolution, and mediation as needed.
  • Participate in all staff meetings, trainings and client meetings as assigned.

Documentation

  • Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards.
  • Correct all documentation as directed by supervisor.
  • Attend all electronic health record (Foothold) training as required.

Qualifications

Education and Work Experience Requirement:

  • High School Diploma or GED and 3+ years working with individuals with developmental disabilities, or an equivalent combination of education and experience that provides the knowledge, skills and abilities to perform job duties and responsibilities proficiently.
  • Associate's Degree or further degree in psychology, mental health, or a related field preferred

Skills/Abilities:

  • The ability to communicate clearly both orally and in writing.
  • Flexible and able to work in a fast-paced environment
  • Self-starter who can innovate new ideas and strategies for helping clients reach goals and outcomes.
  • Patience when working with individuals across a spectrum of intellectual and developmental disabilities.
  • Conflict resolution skills.

 

Special Working Conditions/Requirements

  • Must be 18 years of age or older.
  • Valid New Jersey driver's license is required.
  • Successful clearance of NJ background check.
  • Has fulfilled the necessary training requirements and is in good standing in order to bill Medicaid for Support and Community Care Waiver (CCW) Services.
